Welcome to our practice! All providers are accepting new patients at this time. Please contact the office to schedule a new patient appointment.
New Patient Forms
To prepare for your new patient appointment, you may print and complete the new patient paperwork. Please bring your insurance card, government-issued identification card, current medications list with dosages, a copy of any advanced directives, and pharmacy information to your appointment. If your insurance requires a co-pay, it will be collected at the time of your visit. We encourage you to sign up for our Patient Portal at your first appointment.
Insurance
Insurance participation varies by provider. Please call our office to verify insurance participation and to understand co-payment and referral requirements for your insurance plan.
Patient Portal
Our practice uses an electronic medical record system. As a patient of this practice you, and your designees may access the electronic medical record through the PATIENT PORTAL. You can use the portal to interact electronically with the practice for messages, scheduling, bill payment and other functions. You are also able to view components of your medical record. To request portal access, please contact the practice.
Billing & Payments
If you have provided your insurance information at the time of your visit, we will collect any co-payment from you and bill your insurance company for your treatment. Once your insurance company has provided their share of the payment, you may owe a balance. If so, we will send you a statement indicating the balance due. You may pay the balance via the PATIENT PORTAL or via mail. If you have questions about your bill or, if you would like to develop a payment plan for your balance or co-insurance portion, please call our office and request the Billing Department.
